EA Form "Nama dan Alamat Majikan"

Hi,

When I generate the EA form, i found out that the company name is not shown, and also, there is an additional text behind the address.

how can I add my company name in the first row, and also remove the additional text behind the address?

Like first line is XXX Sdn Bhd

Last line is 41200 Klang, Selangor
